Below are the requirements needed for grades Preschool - 8th

• Complete Online Application/Schedule a telephone appointment
• Provide your current lease or mortgage statement and 3 other proofs of residency (i.e..rent receipt, gas, electricity, cable, water, bank...etc)
• Transfer Card from previous school- Grade K - 8th
• Actual Birth Certificate of child
• Immunizations
• Physical/Dental Forms
• Photo ID
• Residency Affidavits are required if you are living with family or friends and your name isn't on the lease.  The renter must provide a copy of the lease and their photo ID. Unforeseen circumstances may override this requirement. Please speak to a Central Registration staff member.  
• Age Requirements for Preschool:   3 years old before October 1st to qualify for Pre K3, 4 years old before October 1st to qualify for Pre K4, 5 years old before October 1st to qualify for Kindergarten
