Please use the links below to access the health screening forms, in English or Español, that you will need to use daily with your child(ren) prior to sending them to school.  All students will also be visually screened and their temperature will be checked upon arrival to school.

Please remember that staff and students must be symptom free in order to attend school. Attendance will not be negatively affected for those students who are absent from school due to COVID-19 symptoms, but who are still able to participate remotely.
